• Claim Form mentioning the member name and membership number.• Medical report with the exact signs/symptoms and diagnosis• Original stamped breakdown invoice clarifying date and cost of each performed service(s) with a proof of payment (payment receipt – credit card receipt)• Completed payment details section on the claim form shown in the below file. Kindly arrange to send all the completed above documents within six months from the initial treatment date at our office Medmark in Cairo at 44 Abdel Moniem Riad Street-Mohandessin.
